General Information
Registration Form: Group and Chaperone
- This form is to be used by group leaders and chaperones.
Registration Form: Individual Youth
- NOTE: Before downloading and completing this form, youth should contact their parish pastors, parish youth ministers or high school campus ministers to see if their parishes or Catholic high schools are organizing an official group or providing a chaperone for the youth. If so, youth should register through these groups and should not complete this form.
Parental Guardian Liability Waiver and Medical Matters Form
- One of these forms must be filled out by a parent or guardian for each youth attending the pilgrimage. It should be turned in by the parent or guardian to the group leader or chaperone.
